What to Bring to Your Visit
A little preparation keeps your appointment on time. Please bring:
- A photo ID, and your dental insurance card if you have coverage.
- A list of any medications you currently take.
- Your completed new patient forms, or arrive about 10 minutes early to finish them in the office.
